Directions:
From the North: -- take I-5 South, then I-405 South toward Beaverton, then I-26 West

From the South: -- take I-5 North; take the Hwy 217 exit, then I-26 West towards Beaches.

From I-26 W, take the Cornell exit, then turn left over the freeway. At the next light turn left on 158th, then turn left after two stop lights on Schendel and proceed to the left rear building (Athletic Center).

 

Accommodations:   

Homestead Inn 503-690-3600 is within walking distance and is a reasonably priced hotel; Phoenix Inn 503-614-8100; Fairfield Inn 503-972-0048.


Restaurants
: There are many restaurants within a mile of the Activity Center.

 

Current USATT rating system will be used for the purpose of conducting all draws. Unrated players may play in any rated event they want; however, they cannot advance out of rated round-robin events. For players who don't have a USATT or CTTA rating, an estimated USATT rating will be given based on a recommendation by a coach who knows the player or by a visual evaluation of the player by a coach whenever possible.  CTTA ratings will be converted to USATT ratings.

 

We will be using the Enez machine to verify compliance with the new rules. All Semi-Finals and Finals with prize money will be tested.  Players will be required to use ITTF approved rubbers detailed here: http://www.ittf.com/ittf_equipment/pdf/Rubber_List.pdf

 

The USATT dress code will be in effect. See:

Please check in at least 15 minutes before the start of your event.  You need to check in on both days.

 

 

 You must be a current USATT, CTTA, or affiliated ITTF member to play.  USATT Memberships can be purchased with the entry fee.

NOTES:

 

Defaults or disqualifications will preclude a player from receiving any prize money for the specific event.

 

Players who receive $600 or more must fill out Form W-9 (Request for Social Security or Employer ID number)

 

Telephone and email entries, and payment at check-in are at the
discretion of the Tournament Director. Players who pay at check-in
do need to get their entry forms in by November 2nd. Canadian players
will be allowed to pay at check-in.

 

"No Shows" will not receive a refund, unless they have given sufficient notice.

Format: RR denotes Round Robin to Single Elimination.  SE denotes Single Elimination.

All matches are three out of five games to 11 points, except the Open Singles final 8 that are 4 out of 7 games. Entries received after the November 2nd deadline will NOT be accepted.

Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement for the

2011 Pacific Rim Open



1. IN CONSIDERATION of being permitted to participate in any way in
USA TABLE TENNIS Sanctioned Events, I and/or my minor child, our personal representatives, assigns, heirs, and next of kin:

2. ACKNOWLEDGE, agree, and represent that I and/or my minor child understand the nature of Table Tennis Activities and that I and/or my minor child are qualified, in good health, and in proper physical condition to participate in such Activity. I further agree that if at any time I believe conditions or equipment to be unsafe, I and/or my minor child will immediately discontinue further participation in the Activity.

3. FULLY UNDERSTAND that (a) TABLE TENNIS ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by me and/or my child's own actions, or inaction, or the actions or inaction of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I and/or my minor child incur as a result of my participation in the Activity.

4. HEREBY ACCEPT AND ASSUME ALL SUCH RISKS, KNOWN AND UNKNOWN, AND ASSUME ALL RESPONSIBILITY FOR THE LOSSES, COSTS, AND/OR DAMAGES FOLLOWING SUCH INJURY, DISABILITY, PARALYSIS, OR DEATH, EVEN IF CAUSED, IN WHOLE OR IN PART, BY THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW;

5. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE USA TABLE TENNIS, their respective administrators, directors, agents, officers, officials, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I and/or my minor child, or anyone on my and/or my minor child's behalf, makes a claim against any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.

6. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
